When I did the FAST Defenfese ground training, a good portion of time was spent teaching kicking on the ground. The assumption was that if you've been attacked in bed or taken to the ground and both you and your assailant are on the ground, then your legs are going to be stronger than his or her arms. So kicking is a good option.
We did thrusting kicks to a kneeling assailant's thighs to knock him off balance, as well as adductive heel kicks to the face and groin.
