Can anyone send me a student risk disclosure form?

Hi, we're opening a new club and are looking for a good risk disclosure form for our students to sign.

Can anyone point me to one I can use?

You might call them "limited liability" forms or whatever, but its the form that says "Don't sue me if you rip your fingernail".

Hi Rob,

I can send you something but am not a Canadian attorney but it will give you an idea of what we use here in Massachusetts, or at least what I use at my school.

Also Rob your insurance company may provide you with what they want signed as well.

Check with them also.

What would happen if you operate your club without one and someone gets hurt? What's the worst that could happen, just wondering...

It would depend on how your 'club' is organized. Is it a corporation, a sole proprietorship, etc. Also the level of 'negligence' if any involved with the accident. Was a relaease in place................... lots of questions that stem from yours.
